APPOINTMENTS

We see patients by appointment.  If you are unable to keep a scheduled appointment, we ask that you please give us 24 hours notice so that we may give your appointment to someone else in need.

PRESCRIPTION REFILLS OR CHANGES

Due to liability issues, we cannot refill prescriptions or make changes to your medication without an updated appointment. If your prescription is due to run out, please call our office to schedule a follow-up appointment. Thank you!
DIRECTIONS FROM I-85
Take Exit 9, Taylor Road and head south on Taylor Road. Turn left on Vaughn Road. Turn right onto Parliament Place, then turn left onto Parliament Circle. Turn left at the “Parliament Place” entrance. Our office is directly ahead, set back by the tree line. Please call us at 334-416-8298 if you need further directions.

FINANCIAL / INSURANCE

We accept most major health insurance plans.  To confirm that we accept your insurance plan, we encourage you to contact our office to confirm.

At the time of your visit, please be prepared to pay any co-payments, deductibles or non-covered services.  For your convenience, we accept cash, checks and debit/credit cards.

PATIENT REGISTRATION

Upon scheduling an appointment with us, you will receive an email with instructions to access our secure patient portal. You will then be able to complete registration forms online before your appointment. This helps expedite your registration process and decrease your wait time at our office.

For your first appointment, please bring a government-issued photo ID, your insurance card, and a list of medications you are taking, including over-the-counter medications. If you have had an MRI or CT, you will need to bring the CD with you as well. Thank you!
